# Adding an information slide

You can create an information slide at any location in the sequence of questions. For example, you could add an information slide before any questions are displayed to explain the purpose of a questionnaire and how the information will be used or to explain how a quiz will be delivered.

1. On the **FORMS** page, select **EDIT** for the form that you want to add the information slide to.
2. Select the **QUESTIONS** tab, and then select **ADD INFO SLIDE**.
3. In the **Create Slide** window, enter a **Title** and **Text**.
4. Select **SAVE**.
