Creating a form in the web manager

1. Go to https://app.engagevr.io/login, and then log in.

2. On the menu, select Forms .
3. On the FORMS page, select ADD NEW.

4. In the Create Form window, enter a name in the Name field, and then select SAVE.

After you create a form, you can modify the settings and add questions.
